He had the technology. He needed the patients, and a way to run it without him.
Dr. Louis Roncaglione is a general dentist running two practices in the Southeast, Columbia and Charlotte. He built around implants and digital dentistry, investing in equipment most GPs avoid and training specifically to do full arch at volume. The clinical infrastructure was already there.
Two things were not. His front office could not absorb inbound lead volume on top of the existing patient base. And like most GPs stepping into full arch, he had no case-conversion process yet. He looked at other agencies first, and picked TIE for an unglamorous reason.
“They have an on-site call center. These people do this for a living. We do surgery for a living. Let them do their job for a living and we’ll do ours.”
Three changes. Everything else followed.
Three changes. Everything else followed.
Less DSO competition than average, and creative that made the gap obvious. Patients who saw the ads and then researched him converted higher because the authority was established before they called.
Treatment Setters ran the whole pipeline
Across both locations, so his team never came off their core work. He was blunt about why an on-site call center mattered: someone can walk through the door and fix it.
A TC who closes without the doctor in the room
His treatment coordinator now runs every consult. He reviews the scan remotely between procedures, says yes or no on candidacy, and she takes it from there.
19 months. No bad months.

“You were just very straight up with, like, this is what I can do, this is what I can’t. These are some of the things you might see from the other firms, I can tell you that’s really hard to deliver on.”

The doctor became the surgeon again, not the salesperson. That is how full arch scales.
